Prepare the completed private-cremation application, relevant permission documents and contact details. Connect the funeral timetable to the booking only after FEHD confirms the available slot.

Check the deceased's details, applicant and permission

A cremation booking involves more than selecting a date. Names and identification details must agree with the permission documents, and the applicant must be reachable for confirmation. Where a licensed undertaker assists, the family should still know who is applying, who receives FEHD replies and whether the date is confirmed. Prepare the original Cremation Permit, Form 3, or Certificate of Order Authorising Cremation, Form 11, as applicable and as staff instruct. Keep the appointed undertaker's details and the family's acceptable dates together.

FEHB 135: Application for Private Cremation

FEHB 135 is a four-page application form. Obtain the latest official version before completing it; do not guess missing information. Parts requiring the applicant's own completion and signature cannot be signed by the undertaker instead.

At the FEHD counter of a Joint Office

Where the case is handled there, a family representative brings FEHB 135 and the relevant originals for staff to check. If anything is missing, record exactly what is required and who will provide it. Before submission, check the form version, pages, signatures and telephone number. At the counter, present the cremation permission originals and provide requested information. If someone is assisting, clarify authorisation and the undertaker's details. Afterwards, record the confirmed booking, reply channel and outstanding tasks. Submission channels and online-service eligibility may change. Current FEHD requirements take precedence over the experience of another applicant.

Preferred, available and confirmed slots are different

Choose two acceptable date windows where possible. Availability depends on capacity and complete documentation at the time of application.

Government cremation charges and arrangements for a niche after collection of ashes are separate subjects.

Keep one confirmation sheet

One person should maintain the confirmed date, time, location, contacts and remaining tasks. For example, Ms Kwok identifies two suitable dates, her son submits the form and her daughter marks the booking as awaiting FEHD's reply. They notify relatives and update the wake plan only after a slot is confirmed.

Printable pre-submission checklist

Keep death registration and certified-copy requirements separate from documents ready for cremation submission.
